Position Purpose

The Discharge Nurse (RN) is a clinical position that utilizes the nursing process to direct and deliver comprehensive nursing discharge instructions to patients from the discharge suite in a safe and proficient manner according to individual patient needs. This position is responsible for preparing discharge documentation, educating patient on discharge instructions, removing patient peripheral intravenous lines, mitigating any remaining discharge barriers for patients discharging, and answering any discharge questions from patient or family members. The Discharge RN will meet the needs of the patient to help facilitate a timely and efficient discharge from the hospital.

Nature and Scope

This position:

• Facilitates the discharge of patients from inpatient units to the discharge suite.

• Patient discharge instructions are prepared and delivered in accordance with assessed needs from chart review and physician orders.

• Prepares, explains, and validates understanding of discharge instructions by patient and patient’s family members if present.

• Education/instruction are provided specific to the patient’s health care needs and age requirements.

• Documentation is consistent, complete, and thorough. Documentation includes discharge instructions and education resources.

• Assess for presence and remove any peripheral intravenous catheters (no central lines).

• Coordinate any Meds to Beds to be delivered and explained to patient and family if present.

• Ensure patient has received all ordered and approved Durable Medical Equipment (DME) including oxygen prior to discharging patient from the facility. Collaborate with Case Management/Social Work as needed.

• Respond to any patient/visitor in distress and activate appropriate response team.

This position does provide patient care.

Description
Education:
Must have working-level knowledge of the English language, including reading, writing, and speaking English. Appropriate education to obtain and maintain Registered Nursing licensure in the State of Nevada.
Experience:
Preferred minimum two years’ experience in acute setting.
License(s):
Ability to obtain and maintain a State of Nevada Registered Nurse license.
Certification(s):
Current BLS certification by The American Heart Association standards.
Computer / Typing:
Must be proficient with Microsoft Office Suite, including Outlook, Teams, Excel, and Word. Must have the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
